The problem was not knowledge. Staff knew the warning signs. The gap was in the minutes between one shift and the next, when a concern could be mentioned but not owned.
The redesign added one question to every handover and a named owner for every open concern. The evidence is a unit audit, and the record says plainly that staffing changed during the same year.
A concern mentioned is not the same as a concern owned.
